Entry Requirements
- Visa required for all travelers to Madagascar.
- Valid signed passport (include a color copy of the info page).
- At least two blank visa pages (side-by-side); amendment pages are not valid.
- Passport validity: minimum 6 months from the last day of stay.
- Valid, signed U.S. passport.
- One completed and signed Application Form (print, sign).
- Business cover letter from U.S. company (original signature) stating purpose, passport info, and payment of expenses; must be on company letterhead and signed by someone other than the applicant. See sample.
- Multiple entry, up to 90-day stay (as permitted by visa issued).
- Valid, signed U.S. passport.
- One completed and signed Visa Application form.
- Two passport photos.
- Flight itinerary and hotel confirmation.
- Computer-generated itinerary or airline e-confirmation showing U.S. departure and arrival/departure to/from Madagascar (with valid record locator), or travel-agent letter on company letterhead confirming booking.
Photo Requirements
- Your head must face the camera directly with full face in view.
- Neutral facial expression or natural smile, with both eyes open.
- Taken in clothing normally worn on a daily basis.
- Taken in the last 6 months.
- Plain white or off-white background.
- 2 x 2 inches (51 x 51 mm).
- Head between 1–1 3/8 inches (25–35 mm) from chin to top of head.
- Printed on matte or glossy photo quality paper, in color.
- No glasses.
- No hat or head covering (exceptions for documented religious or medical reasons).
- Full face visible; no shadows on face.
- No headphones or wireless hands-free devices.
Health Requirements
- Yellow fever: vaccination certificate required if arriving from (or transiting) an affected area within 6 days prior to entry.
- Cholera: vaccination may be recommended when arriving from an infected area (cert valid rules per issuing authority).
- Hepatitis A: vaccination recommended for international travelers.
- Malaria: year-round risk — consult your physician for prophylaxis.
