Entry Requirements
Must submit a computer-generated itinerary from a travel agency or online travel site. E-confirmation from an airline or travel company showing departure from the U.S. and arrival/departure to and from your visa destination. Acceptable with a valid record locator.
Photo Requirements
- Your head must face the camera directly with full face in view.
- Neutral facial expression or natural smile, with both eyes open.
- Taken in clothing normally worn on a daily basis.
- Taken in the last 6 months.
- Plain white or off-white background.
- 2 x 2 inches (51 x 51 mm).
- Head between 1–1 3/8 inches (25–35 mm) from chin to top of head.
- Printed on matte or glossy photo quality paper, in color.
- No glasses.
- No hat or head covering (exceptions for documented religious or medical reasons).
- Full face visible; no shadows on face.
- No headphones or wireless hands-free devices.
If Traveling for Business
Note: Self-employed must also submit a copy of a recent bank statement.
- Applicant must submit a Business letter of support from a U.S.-based company.
- This letter must be on your company letterhead.
- This letter must be signed by someone other than the applicant.
- Include passport information.
- State how your expenses will be paid.
- Must be an original signed document. (Some destinations also require passport information in the letter.)

If Traveling for Tourism
- Tourism visa requires financial proof of support.
- Bank statement and letter of intent.
Health Requirements
- International Certificate of Vaccination for Yellow Fever must be presented with application request.

Accompanying Minors
- Application form signed by both parents or legal guardian(s).
- Notarized letter of consent signed by both parents or legal guardian(s).
- Original and copy of the minor’s birth certificate.
Non-U.S. Citizens
- If you’re a United States Alien resident, a copy of your Green Card must be attached to your application.
- If you’re a United States Visa holder, a copy of your U.S. Visa must be attached to your application.
Additional Information
Valid Signed Passport
- Applicant must submit their current signed passport. Applicant must also include a color copy of the information page of the passport.
Note: In order to obtain visas, passport must have at least two blank visa pages side by side. Pages in the passport marked “Amendment” are not valid for visas. JIDS recommends your passport is valid for at least six months past the last day of stay from international destination.
