Entry Requirements
- Passport and Visa are required.
- Valid signed passport
- Applicant must submit their current signed passport.
- Note: Passport must contain at least two blank visa pages side by side. “Amendment” pages are not valid for visas.
- JIDS recommends at least 6 months’ validity beyond your last day abroad. Assistance available: passport services.

- Two application forms, both duly signed by the applicant (copies accepted).
- Photo Requirement
- Two (2) color passport-style photographs (2×2), recent within 6 months, different from passport photo.

- Letter from employer confirming return to U.S. to resume duties or letter from airline/travel agency guaranteeing onward return ticket.
- Copy of vaccination certificate.
- Invitation letter from host in Central African Republic.
- Proof of Travel
- Computer-generated itinerary from a travel agency or airline e-confirmation showing U.S. departure and valid record locator.

Photo Requirements
- Your head must face the camera directly with full face in view.
- Neutral facial expression or natural smile, with both eyes open.
- Taken in clothing normally worn on a daily basis.
- Taken in the last 6 months.
- Plain white or off-white background.
- 2 x 2 inches (51 x 51 mm).
- Head between 1–1 3/8 inches (25–35 mm) from chin to top of head.
- Printed on matte or glossy photo quality paper, in color.
- No glasses.
- No hat or head covering (exceptions for documented religious or medical reasons).
- Full face visible; no shadows on face.
- No headphones or wireless hands-free devices.
Health Requirements
- No vaccinations required unless arriving from or transiting an infected area.

Accompanying Minors
- Application signed by both parents/guardians.

Non-U.S. Citizens
- Include a copy of your U.S. Green Card (permanent residents) or your U.S. visa (visa holders).
